PATIENT HISTORY
Sam Winchester was the second child to the lovely couple John and Mary Winchester, he is Dean's younger brother. When Sam was just six months old his mother died by the demon Azazel, Mary walked into Azazel when he was feeding Sam his demon blood. Once their mother's body was found, the ceiling turned on fire, John gave Sam to Dean who ran out of the house instantly once Sam was given to him while their father was in the house, trying to save their mother but it was too late. The two Winchesters spent years travelling from town to town, from school to another. From a young age, Sam knew that his mother died in a car accident, or that's at least what he had thought and their father is a travelling salesman. One night, Sam found his father's journal and went straight to Dean, demanding him to explain everything that is written in the Journal. Dean revealed to him the existence of the supernaturals. By the age of eleven, Sam spent his Thanksgiving day with another family, for him It was the most realistic Thanksgiving. The way he used to spend it with Dean was that they eat a crispy chicken while they father would pass out on the couch. Sam started to go hunting with Dean and his father by such a young age, only by the age of twelve. Soon enough the young boy started to crave a normal life, away from monsters and away from hunting. When a teacher of his, unaware of the whole story, encouraged him to seek "the family business."
One of the fourth of July, both Dean and Sam snuck out to a field with fireworks and they started to play with the fireworks and burned down the field by a mistake. Sam was usually left by Dean at Plucky Pennywhistle's Magical Menagerie while Dean hangs out with girls which caused Sam to have phobia of clowns, fearing them to death. When Sam reached the age of Fifteen, he was left to hunt a Kitsune alone and while doing researches at the library, he met Amy. He protected her from two boys and violently ended up with a bloody fight and of course, Sam won and he met Amy properly who invited him over to her house to help him with his cheek from the rough punch. Sam never knew that Amy was the Kitsune's daughter, but he soon figured out after kissing Amy and hiding in the closet once her mother suddenly knocked on the door. He saw the brain and wanted to kill Amy but Amy's mother returned and grabbed Sam, willing to kill him and just in time, Amy had stabbed her mother in the back with the knife that Sam dropped, killing her mother and saving Sam from her mother. Sam grew up with Dean and left the family for Standford University while their father went missing right after when he went on a hunt to find the thing that killed their mother. Sam was finally having the normal life that he always wanted with a girl named Jessica, but he managed to let her remain in the dark about his history. He went on a hunting mission with Dean only to come back to find his girlfriend killed in the same way their mother was killed with and that's when Sam decided to leave his plans and go on a hunt with Dean to find their father and that's when all the hunting started.
The two brothers faced lots of bumps on the road, but luckily the managed to go through but not everyone survived. Their father died, John after they found him and Sam had his issues with his father and they both weren't on the bed terms and Sam still feels guilty because his father died and he never got the chance to let out a proper apology towards his dad. Sam was possessed for a short time by a demon, Dean knew if he got a good chance he will have to shoot his brother but he never did so and Sam while being possessed by a demon, he shot another hunter, killing the person. Sam was transported into an abandoned town with the chosen children by Azazel and by the end, one of them, stabbed Sam in the back, killing him. Dean set off a deal with a demon for Sam to come back to life And that Dean sold his soul for his brother. Sam tried his best to set off the deal, but it was too late, the hellhounds ripped his brother in front of his eyes and he watched his brother die.
After Dean's death, Sam cuts all the connects he had with Bobby, still, Sam keeps on trying to find a way to bring Dean back to the land of the living, once more. It all stopped when he found his brother at the door step while Ruby was in the same room but in a different vessel and of course, Sam didn't let Dean know about it. Sam attacked Dean thinking that he's a monster taking the shape of his brother, but he was proved wrong by Bobby. Sam started to help Dean to find out who pulled him out of hell and why. The road was long and full of sudden events and tricky moments, but the two finally figured out. It was an angel. Sam was a believer, so he took the words in quickly and believed Dean once he had told him about Castiel. Sam also promised his brother to stop using his powers and it grew by the help of Ruby, the demon. The more demon blood Sam drinks the more powerful he gets. Dean warned him many times about the dangerous road he's heading down to and that Castiel will hunt him down if he doesn't stop. Sam was a head over heels determined to kill Lilith for killing Dean. Revenge. He was manipulated by Ruby to free Lucifer from the cage and It worked, once it did and Lucifer was free to walk the earth, Sam helped Dean kill Ruby.
Due to breaking the final seal and freeing Lucifer from hell, Sam was being influenced strongly by demonic powers but God transported Sam and Dean onto a plan and saved them, turning Sam Winchester into a human again. Sam had plans to stop Lucifer and his devious plans, just like how he started the war, he selflessly ended it by throwing himself with Michael into the cage. Sam made Dean promise to not bring him back from the cage, It was too risky. Sam returned mysteriously from the cage and once again back to the land of the living, he tried to contact Castiel but he never answered his prayers. Sam watched his brother and also, hunted with a group and the leader was his grandfather, Samuel. Sam came out of the cage with no soul, he had no sympathy, no emotions and he didn't feel anything. He was ruthless and driven. He did what he had to do to get his job done, even killing innocent people. It was soon revealed to everyone that Sam had no soul and he pleaded for Dean's help to help him get his soul back, once again but he was just fooling Dean, he never wanted his soul back and that's when he got help from Baltazar, telling him to scar his vessel and by scarring means, killing someone very close to Sam which was Bobby Singer. But Dean got in time when he was about to kill Bobby, Death had pulled Sam's soul from hell and placed a wall in Sam's mind and warned him not to scratch it.
Sam was once again, normal and human with a soul, but it didn't last long before Castiel broke the wall inside Sam's mind and he started to remember everything and most likely, remember his time in hell which everyone was afraid of. Sam was out for a few days before he came back only to find out that Castiel was the new God and Sam slowly started to lose his mind as he started to see Lucifer everywhere and every day but Sam had a scar in his hand which helped him free his mind from Lucifer for a couple of minutes and even hours if he's lucky. Sam only let Lucifer in once when he helped him with a case and that's when he made a terrible mistake by letting Lucifer in his head, that's when the real haunting started for the young Winchester. He couldn't sleep, he couldn't eat and he can barely focus on the simplest tasks. He was out, running away from Lucifer into a mysterious place and that's when Lucifer followed him again and made him panic. He even took drugs for the first time, hoping that it would be enough to knock him out for at least an hour to rest his eyes. But it didn't work, it didn't stop Lucifer. Running away, again, Sam passed the green light and got hit by a car.
He was soon moved to Eichen house by the hospital for his state. The doctors saw Sam talking to himself and staring at the wall, calling someone by the name 'devil' and he talked to himself over and over again. It was just a matter of time before Dean would find Sam and take him out of that place. He hoped so.
